Vista Care provides specialized, individualized services that are designed to assist individuals with a Traumatic or Acquired Brain Injury live a full and productive life, as independently as possible. The consequences of Traumatic & Acquired Brain Injury often require a major life adjustment around the person’s new circumstances for the individual as well as their family. An interdisciplinary team works together to identify the individual’s service needs
and desired personal outcomes. The needs, goals and desires are written into a specialized service plan tailored to help the individual live as fully and independently as possible while receiving intensive support and services.

RESIDENTIAL SERVICES
At Vista Care, Individuals’ choices, dignity, and rights are a priority when developing individualized support plans. Each potential admission to one of our residential homes is professionally assessed and reviewed by an interdisciplinary team to develop the highest quality services and supports needed for each individual. Unlike other providers of service, we offer a home, not a home-like atmosphere. A home is an integral part of the quality of life for all all of us. We are advocates of each individual's autonomy, rights and choices. We enable persons to satisfy these needs by providing support, assistance, education, encouragement and an atmosphere that enhances the quality of life for adults and families managing challenges associated with Traumatic & Acquired Brain Injury. We provide optimal community involvement and full inclusion. The environment is structured to assure proper supervision, comfort and safety. Vista Care consistently collaborates with each individual, the individual's guardian, and the case worker to ensure the changing needs of each person we serve are addressed.
